What are the requirements in applying for license/permit?

1. Professional Basketball

a. Two (2) copies 2″x 2″ colored ID PICTURE (White background)
b. X-ray, ECG, Drug Test and Hepa-B result
c. Passport number/Community Tax Certificate Number
d. Special Working Permit (for imports only)
e. DOJ clearance (for Fil-Am only)
f. PBA uniform Contract
g. ITR (photocopy) – Promoter only
h. Assets and Liabilities – Promoter only (Case to case basis)

2. Other Sports (which include Golf, Billiards, Cycling, Tennis, Volleyball, etc.)

a. Two (2) copies 2″x 2″ colored ID PICTURE (White background)
b. Medical requirements (ECG, X-ray, Drug Test)
c. Passport number/Community Tax Certificate Number
d. ITR (photocopy) – Promoter only
e. Assets and Liabilities – Promoter only (Case to case basis)
f. Certificate of membership from Association/Club (For new applicants only)

3. Cockfighting

a. Copy of city/municipality license
b. Copy of police clearance
c. Certificate from cockpit owner where the applicant is presently employed
d. Two (2) copies of 2″ x 2″ colored ID picture (White background)
e. Cash Bond deposit in the amount of P200,000.00, withdrawable after the event (for Promoters of International Derby only)

 

Life Span of the License

As per GAB Memorandum circular No. 2013-01, the expiration of licenses of all professional sports and games licensees is hereby extended up to March 31 of every calendar year.

 

Renewal of License

Licenses must be renewed annually not later than the last day of March of every calendar year. A penalty of fifty percent (50%) of the basic license fee shall be imposed on late renewals. If the last day of March falls on a non-working day of special or legal holiday, licenses may be renewed without penalty on the next working day.

 

After the last day of March of every calendar year, all licenses are deemed expired. All athletes will be required to renew their licenses before they will be allowed to participate in any promotion.

 

Athletes who are scheduled to participate in a promotion abroad shall submit the usual medical requirements/invitation from promoter before they will be issued Travel Authority.
